- Confirm you have download the PDF of form 186 from the ECFMG interactive web application IWA and have a copy of the original document
- Create a color image of the required pages of your passport (page with your picture on it)
- Confirm your name on the form 186 is correct and matches your passport exactly.
- Go to the NotaryCam website at www.notarycam.com/ecfmg
- Click on the “CERTIFICATION 186 IDENTIFICATION FORM”
- Select YES to questions 1, 2, 3 then select continue.
- Upload the pdf of form 186
- Upload the passport or ID to be used.
- Enter your ECFMG #
- Select YES for option 4
- Select YES for option 5, then select continue
- Perform the TECH CHECK to verify your camera and microphone are compatible.
- Follow the prompts to verify your identity using your preferred document.

- A few minutes before the appointment time you will receive the link.
- Have your official passport, adequate light and computer with web cam compatibility.
- After 15 days, ECFMG will confirm your form has been completed and you can now apply for the ECFMG exams.
